To become enrolled in eShay.com's business opportunity, I will need to fill out the form below. The cost for enrolling in this program is a low one-time fee of $29.99. This is the only fee that I will ever pay as a part of this program. This is guaranteed in writing. Acceptable forms of payment can be found on the form below. This fee is refundable to me up to 30 days after my dated enrollment form. It is important that all fields are completed.
